Assessment and prescribing process
The assessment and prescribing process outlines four steps:
Assess > Plan > Implement > Review
The process supports clinical reasoning and shared decision-making with the person and, where relevant, their family, carers or support network. It includes templates that can be adapted to individual needs.
Motor Vehicle Transport Plan
The Motor Vehicle Transport Plan provides a key record of the person’s motor vehicle transport needs, agreed strategies and prescribed supports. It can be shared with family members, carers, support workers, schools, services and other professionals involved in the person’s transport.
Transport Safety Advice Form
The Transport Safety Advice Form documents the prescribed vehicle restraint or device, the review period, and relevant information about installation and safe use. It may also support road-law requirements in some states and territories.
