Eligible people are those who are unable to walk and always require the use of a wheelchair, have their ability to walk severely restricted by a permanent medical condition, or ability to walk is severely restricted by a temporary medical condition, or disability that they will have for 6 months or more, as well as those diagnosed as 'legally blind'.

Apply online, or in person or by post with completed form. Provide medical evidence in the form of 'Part 2 - Medical Details' of the application, which can be completed by your doctor or occupational therapist. Lodge your application online or at your nearest Transport and Motoring Customer Service Centre, or mail.

An intellectual, psychiatric, cognitive or sensory impairment alone does not meet the eligibility criteria, unless you have an eligible mobility or vision impairment.
