South Australia

You are eligible if you have a permanent physical impairment or a temporary impairment likely to continue for more than six months that severely restricts speed of movement and significantly inhibits the ability to access public transport. Legally blind applicants are included, and they needn’t meet further mobility criteria. A legally blind person being transported in a vehicle is eligible and doesn’t need a driver’s licence. Applicants with a significant intellectual, cognitive, behavioural or neurological impairment are included and they must be unable to mobilise without the continuous support of another person. The previous requirement that the impairment must significantly inhibit an applicant’s ability to use public transport has been removed.

After checking eligibility, applicants should download the application form and complete their section (a parent or guardian can complete it if the applicant is unable to). A doctor must then complete and sign the relevant sections in full, before the applicant submits the form in person at a Service SA centre, or by post. There is no option to submit online.
